File A Report (Title IX)
FILE A REPORT
Please use this form to report sexual misconduct, sex discrimination, gender-based discrimination, sexual harassment, sexual assault, dating violence, domestic violence, stalking, or other Title IX related allegations.
REPORTING OPTIONS
Title IX concerns should be made to the Title IX Coordinator or to other administrators who report to the Superintendent. Reports can be made by phone, email, website online report form, or in person. Reports may also be made anonymously. However, employees who are responsible employees (mandatory reporters) and may not file a report anonymously. Reports will be forwarded to the appropriate office.
Victim/survivors have the right to report without further participation in any investigation. If a victim/survivor chooses to report, a university authority can assist. The District will make available interim measures, such as accommodations, regardless of whether the victim/survivor chooses to report to police or to Era ISD.
Retaliation for filing a report or participating in an investigation is not tolerated at Era ISD.
CONFIDENTIALITY
The District will do its best to honor the victim/survivor's request for confidentiality, however, confidentiality is not guaranteed. Most District employees are considered "responsible employees" meaning they are mandated to report Title IX allegations to the Title IX Coordinator. The District may need to act to maintain campus safety and must determine whether to investigate further, even if the victim/survivor requests to remain confidential or requests the District not to investigate further.
While a victim/survivor’s conversation will be kept private (will not be shared unnecessarily with others), only a conversation with a confidential resource can be kept confidential (meaning a victim/survivors identity will not be shared with anyone without the express written consent of the victim/survivor).
Confidential resources are still required to report non-personal identifying information about the incident to the Title IX Coordinator. Confidential resources may include Counseling and Health Services in certain situations.
