Parents of 7th grade students must provide proof of their student having received their Meningococcal, Varicella, and Tdap vaccine by August 1st, 2016.
Students who do not have current immunizations on file at the school, will not be able to start school until those immunizations are complete.
Parents of kindergarten students must have provided an up to date immunization record by August 1st, 2016, including 2nd Hep. A, Varicella, and MMR. Kinder students must also have received a Dtap and Polio booster after their 4th birthday.
Immunization records may be attached and emailed to firstname.lastname@example.org or brought to the elementary or HS office.
